GPS Health Navigators is a new not-for-profit virtual navigation clinic that has been registered to support refugee patients on the Interim Federal Health Program (IFHP) with palliative care needs in the Greater Toronto and Hamilton Region (GTHA). This service has been established based on explored barriers and facilitators to providing palliative care for refugees, specifically when transitioning patients from acute care settings to the community.
