About us
GPS Health Navigators is a new not-for-profit hybrid (virtual and on site) navigation clinic that has been registered to support refugee patients on the Interim Federal Health Program (IFHP), via Blue Cross. We aim to specifically support those with palliative care needs in the Greater Toronto and Hamilton Region (GTHA). This service has been established based on explored barriers and facilitators to providing palliative care for refugees, specifically when transitioning patients from acute care settings to the community.
How GPS works
This service involves a social worker (SW) and Palliative Care Specialist who will become a key point of contact and meet with refugee clients at the point of referral and throughout the patient's disease trajectory. The team would make efforts to connect patients with nursing, PSW, and social services in addition to facilitating coverage for important medications and medical equipment from Blue Cross/IFHP. The SW would check in regularly to support in modifying services, equipment, and medications, according to the patient's evolving needs and functional status. This service would aim to mirror provincially funded home care for OHIP-covered patients.
